Youth to Adult Disability Healthcare

This was a consultation project from the Holland Bloorview Paediatric Hospital for children with disabilities in Toronto.
Due to shortcomings in provincial legislation, youth with disabilities have an incredibly difficult time finding family doctors as they become adults. Our research and strategies looked deeper at why this was happening, what stakeholder groups could help remedy this, and what strategies Holland Bloorview could employ over the next 5-10 years to better support their patients as they exit the hospital.

Our recommendations looked at reducing disability stigma with adult family doctors, collaborating with future doctors in training, and advocating to extend youth care with the Ministry of Health.
